To request an appointment at Parker Sports Medicine & Orthopedics, you can contact us at (806) 350-2663 and have one of our admitting personnel assist you.
To facilitate the appointment process, please have your insurance card available as we will ask you about your coverage.
If your insurance plans have office visit co-pay, you will be expected to pay the co-pay at the time of your visit.
We require that you bring any recent X-Rays, MRIs or CT scans with you to your appointment. You will have to contact that facility where the procedures were performed and sign a release for the films. We would like to have the film and reports. Without those films, we may have to reschedule your appointment so that we do not duplicate diagnostic tests. In some cases, the films or techniques are not adequate and may need to be repeated.
You may come to the office approximately 30 minutes before your appointment time and complete requisite paperwork or you can download the forms below, complete them, and bring them with you at the time of your appointment.

You, as the responsible party, must furnish our office with the up-to-date insurance information. A copy of your insurance card will be made. Report any changes in insurance coverage immediately upon arrival. Any balance due, not covered by insurance, is your responsibility and is due upon receipt of statement from our office.
Self-Pay Patients or No-Insurance
If you have no insurance and/or have experienced hardship, ABJC has developed an affordable policy in which you can utilize:
• An office visit in which an x-ray is needed is $250.
• An office visit in which an x-ray is not needed is $200.
• There may be additional charges after being seen by your physician. These charges will be provided to you upon check out and payment plans are available. These additional charges could include casts/splints/injections/additional x-rays/procedures.
If you need to set up a payment plan, please call the billing office to discuss or set it up at your appointment in person.
Past due accounts
For accounts not paid in a responsible manner, we block their account and they are unable to make future payments until the balance is discussed or current. We expect it to be paid in a timely manner to avoid being sent to collections.
